Intensive Support Team (Learning Disability)
Description
The Intensive Support Teams (IST) provides specialist assessment and assertive interventions for young people and adults with a learning disability aged 16 years and above, who are at immediate risk of placement breakdown or hospital admission due to having complex health needs including behavioural distress and/or mental health deterioration.
IST works as part of a multidisciplinary team alongside community learning disability services, education and local authorities. We work in collaboration with service users and their family / carers to understand the reasons for the potential breakdown
The service offers assertive intervention and practical support to reduce the risk of family / placement breakdown.
The team works 7 days per week, 8am-8pm.
